Leave of Absence
A leave of absence request must be completed if an employee will be absent for six or more consecutive days. The request for a leave of absence should be completed and submitted using the Mesquite ISD Request for Leave form. Leave requests without all required documentation will not be approved.
A physician’s statement is required for leave approval. You may upload the documentation to the Google Form Request, email or fax a copy of your physician's statement to Brenda Barron in Personnel Services.

To qualify for FMLA, a Mesquite ISD employee must have been employed for 12 months and have worked 1,250 hours in the past 12 months.

A medical statement from a licensed certified medical provider containing the same information as an FMLA packet will be accepted.

Classroom teachers, paras and other specific student-centered positions will receive an additional document to complete and return prior to approval of your leave of absence. This form will be sent once all other required documentation has been submitted and the request form is complete.

Provide military orders and training schedule to Brenda Barron in advance of any absences.

Employees on an approved medical leave are required to schedule a Return-to-Duty meeting with Personnel Services at least 30 days prior to their anticipated return date. To schedule this meeting, please click the button below. At the time of your meeting, you must provide a physician’s statement confirming that you are medically cleared to return to work.

- Jury Duty
- Comp. Time
- Vacation
- Non-Duty
- School Business
- Employee is out of State & Local Days
- Black Out Days
